Centurion is seeking a Part-time Day Shift Discharge Planner to join the Substance Use Disorder (SUD) team at Eastern Correctional Institute in Westover, Maryland. The role focuses on coordinating releases from the SUD program, building referrals with community services, and ensuring smooth transitions upon discharge.

The ideal candidate holds a Bachelor’s in Psychology, Social Work, Counseling, or related field (Master’s preferred) with at least 2 years in release planning.

Qualifications

• Bachelor’s Degree in Psychology, Social Work, Counseling, or related field from an accredited educational institution; Master’s Degree preferred
• Minimum of two (2) years of experience working in release planning preferred
• Clear written and verbal communication skills
• Must be appropriately and actively certified in Cardio-Pulmonary Resuscitation (CPR)
• Corrections, forensic or criminal justice experience preferred
• Ability to obtain a security clearance, to include drug screen and criminal background check

Responsabilités

We are currently seeking a Part-time Day Shift Discharge Planner to join our Substance Use Disorder (SUD) team at Eastern Correctional Institute in Westover, Maryland.

The SUD Program Discharge Planner is responsible for coordinating a patient’s release from the substance use program upon release or leave to another facility. This person is responsible for creating and maintaining relationships with referral sources and community agencies providing services to persons with substance abuse and co-occurring mental health disorders, as well as, identifying the need to return patients to referral source at the time of discharge.

Shift: Part Time Day 20 hours per week

Qualifications

• Bachelor’s Degree in Psychology, Social Work, Counseling, or related field from an accredited educational institution; Master’s Degree preferred
