Job summary

VitalCore Health Strategies in Wilmington, Delaware, seeks a Reentry Specialist with SUD experience to coordinate community reintegration for individuals leaving incarceration. This full-time role focuses on connecting clients with services, advocating for needs, and ensuring continuity of care after release.

Qualifications include a Bachelor’s in Social Work, Counseling, Psychology, or related field, plus experience with substance use disorders and case management.

Qualifications

• One year of experience in social services field.
• Bachelor’s degree in Social Work, Counseling, Psychology, or related field.
• Experience working with individuals with Substance Use Disorders.
• Case Management experience.
• Applicants with experience in a correctional health care setting will be given preference.
• Certification or credential in substance use counseling (e.g., CADC, CSAC, or equivalent) strongly preferred.
• Knowledge of SUD treatment and recovery principles.
• Strong communication, organizational, and advocacy skills.
• Ability to work effectively within a multidisciplinary team.

Responsibilities

• Provides access to community programs and ensures continuity of care after release.
• Advocates for patient needs and coordinates with multiple systems affecting offenders with mental health issues and SUDs.
• Acts as liaison among healthcare, correctional, and community services to support reentry goals.

REENTRY SPECIALIST POSITION SUMMARY:

The Reentry Specialist provides access to community programs, ensures continuity of care following release into the community, advocates for patient needs, and act as liaisons between the multiple systems impacting the lives of offenders with mental health illnesses, co-occurring substance use disorders, and chronic medical illnesses.

REENTRY SPECIALIST MINIMUM REQUIREMENTS:

• One year of experience in social services field.
