Centurion is seeking a Part-time Day Shift Discharge Planner at Eastern Correctional Institute in Westover, MD, to coordinate releases from the SUD program and ensure smooth transitions.

The role emphasizes building relationships with referral sources, community agencies, and coordinating follow-up care; CPR certification is required, and a bachelor’s in a related field is required with a master’s degree preferred.

Qualifications

• Bachelor’s degree in Psychology, Social Work, Counseling, or related field from an accredited institution.
• Master’s degree preferred.
• Minimum of two years of experience working in discharge planning.

Responsibilities

We are currently seeking a Part-time Day Shift Discharge Planner to join our Substance Use Disorder (SUD) team at Eastern Correctional Institute in Westover, Maryland.The SUD Program Discharge Planner is responsible for coordinating a patient’s release from the substance use program upon release or leave to another facility.
