Job summary

Centurion is seeking a full-time RN Discharge Planner at Maryland Correctional Institution for Women in Jessup, MD. The role focuses on coordinating discharge planning within a correctional setting and ensuring continuity of care after release.

The Discharge Planner will work under the Director of Nursing with a multidisciplinary team to develop safe discharge plans and promote patient well-being from incarceration to community care.

Qualifications

• Current Registered Nurse licensed in Maryland or able to obtain license.
• BLS and CPR certifications required.
• 1 year of experience as a Discharge Planner or Case Manager.
• Corrections or acute setting experience preferred.
• Ability to obtain a security clearance including drug screen and background check.

Responsibilities

• Coordinate patient discharge planning from a correctional setting.
• Collaborate with the Director of Nursing and multidisciplinary team.
• Develop and implement a safe discharge plan to promote continuity of care after release.

Centurion is proud to be the provider of comprehensive healthcare services to the Maryland Department of Public Safety and Correctional Services. We are currently seeking a Full-Time RN Discharge Planner to join our team at Maryland Correctional Institution for Women located in Jessup, Maryland.

The RN Discharge Planner is responsible for coordinating patient discharge planning from a correctional setting. The Discharge Planner will work under the direction of the Director of Nursing in collaboration with a multidisciplinary team to develop and implement a safe and appropriate discharge plan to promote continuity of care after release.

Qualifications

• Current Registered Nurse in the state of Maryland or the ability to obtain license required
• Current BLS and CPR certifications required
• On-year experience as a Discharge Planner or Case Manager required
• Experience in corrections or an acute setting preferred
• Ability to obtain a security clearance to include drug screen and criminal background check
