Job summary

Centurion Health in Baltimore, MD is seeking a Full-Time RN Discharge Planner to coordinate patient discharge planning from a correctional setting at the Baltimore Central Booking & Intake Center. Under the direction of the Director of Nursing, you will collaborate with a multidisciplinary team to develop safe discharge plans and promote continuity of care after release.

Requirements include MD RN license, BLS/CPR, and at least one year of discharge planning or case management experience;

Qualifications

• Current Registered Nurse license in Maryland or ability to obtain
• BLS and CPR certifications
• One year of experience as a discharge planner or case manager
• Corrections or acute care experience preferred
• Ability to obtain security clearance including drug screen and background check

Responsibilities

We are currently seeking a Full-Time RN Discharge Planner to join our team at BCBIC (Baltimore Central Booking & Intake Center) located in Baltimore, Maryland.

The RN Discharge Planner is responsible for coordinating patient discharge planning from a correctional setting. The Discharge Planner will work under the direction of the Director of Nursing in collaboration with a multidisciplinary team to develop and implement a safe and appropriate discharge plan to promote continuity of care after release.

Qualifications

• Current Registered Nurse in the state of Maryland or the ability to obtain license required
• Current BLS and CPR certifications required
• On-year experience as a Discharge Planner or Case Manager required
• Experience in corrections or an acute setting preferred
• Ability to obtain a security clearance to include drug screen and criminal background check
