Centurion is seeking a Part-time Day Shift Discharge Planner at Eastern Correctional Institute in Westover, MD, to coordinate releases from the SUD program and ensure smooth transitions.

The role emphasizes building relationships with referral sources, community agencies, and coordinating follow-up care; CPR certification is required, and a bachelor’s in a related field is required with a master’s degree preferred.

Qualifications

• Bachelor’s degree in Psychology, Social Work, Counseling, or related field from an accredited institution.
• Master’s degree preferred.
• Minimum of two years of experience working in discharge planning.

Responsabilités

• Coordinate a patient’s discharge from the SUD program upon release or leave to another facility.
• Create and maintain relationships with referral sources and community agencies.
• Identify need to return patients to referral source at the time of discharge.
