Centurion is seeking a full-time RN Discharge Planner for Maryland Correctional Institution for Women in Jessup, MD. You will coordinate discharge planning from a correctional setting under the guidance of the Director of Nursing and work with a multidisciplinary team to ensure safe, effective discharge plans that promote continuity of care after release.

MD RN licensure with BLS/CPR is required, and experience in discharge planning or case management is preferred.

Qualifications

• Active Registered Nurse license in MD (or ability to obtain)
• Current BLS and CPR certifications required
• Experience as a Discharge Planner or Case Manager preferred

We are currently seeking a Full-Time RN Discharge Planner to join our team at Maryland Correctional Institution for Women located in Jessup, Maryland.The RN Discharge Planner is responsible for coordinating patient discharge planning from a correctional setting.
