DESC is seeking a Substance Use Disorder Professional (SUDP) for its WellConnect program. The role focuses on leading the SUD program, coordinating treatment plans, and providing both office and community-based care for PSH clients.

The position requires a WA SUDP license and strong collaboration skills. The WellConnect team emphasizes wrap-around services, 24/7 crisis coverage, and integration with DESC’s housing and behavioral health services.

Formación

• Licensed by the State of Washington as a SUDP in good standing.
• Active SUDP license issued by the Washington State DOH.

Responsabilidades

• Manage and facilitate WellConnect’s SUD Programming.
• 80% facilitation of the SUD program and 20% direct case management work.
• Update substance use treatment plans monthly according to stage of change.
• Act as the WellConnect team's subject matter expert for SUDs.
• Provide consultation to team members on SUD issues and Harm Reduction practices.
• Provide staff training on substance use issues as appropriate.
• Provide individual and group SUD treatment in office and community settings.
• Plan, organize and facilitate treatment and support groups for clients.
• Monitor and coordinate replacement of harm reduction supplies.
• Advocate for client access to community resources and services.
• Develop and maintain collaborative relationships with DESC staff and other providers.
• Participate in psychiatric consultation, supervision, and team meetings.
• Maintain complete clinical records and comply with QA procedures.
• Participate in on-call rotations for 24/7 crisis coverage.
