Job summary

Seattle Roots Community Health is hiring a Substance Use Disorder Professional (SUDP) to deliver outpatient SUD treatment services. You will assess, diagnose, counsel, and coordinate care for patients with substance use disorders, collaborating with staff and school-based sites to support youth and communities.

The role emphasizes harm reduction and patient-centered care within a not-for-profit urban health system.

Qualifications

  • Active WA SUDP license required.
  • Minimum 2 years of experience in the field.
  • Experience with harm reduction philosophy and adolescents.
  • Valid driver's license and CPR certification.
  • Proficient with EMR systems (EPIC) and office tools.

Responsibilities

  • Serve as a contact for medical providers on SUD-related issues.
  • Perform SUD assessments and make treatment recommendations.
  • Provide care coordination and rapid placement into appropriate care.
  • Develop relationships with community resources.
  • Counsel patients individually or in groups to support recovery.
  • Maintain accurate records of patient histories and progress.
  • Develop and evaluate treatment plans based on evidence and history.
  • Provide information and referrals for SUD services to patients and families.
  • Plan follow-up or aftercare for discharged patients.
  • Collaborate with school-based sites to support youth and communities.
  • Counsel patients and support systems on harm reduction methods.
  • Attend Addiction Medicine Program meetings and coordinate accordingly.
