Job summary

Santa Rosa Community Health seeks a Substance Use Disorder Case Manager to manage a dedicated caseload within an integrated care setting. You will assist with 1:1 visits, warm handoffs, assessments, treatment planning, referrals, and data entry in the EHR, coordinating with MAT and IBH teams to support patient engagement and retention.

Qualifications

• AA degree (Bachelor’s degree preferred) or two years of work experience in a social/human service setting.
• Current certification as an AODS counselor through an organization accredited by the State of California or registration as an AODS counselor intern with capability to complete AODS counselor certification within two years.

Responsibilities

• Assist with meeting the needs of SUD/MAT patients, including 1:1 medically supervised visits and warm handoffs.
• Create and update treatment plans, care coordination, follow-up care and outside referrals.
• Document patient information, monitor UTOX results, and communicate progress to medical providers.
