Qualifications

• Minimum of two years of experience in behavioral health.
• Strong knowledge of community resources and relationships with local agencies.
• Ability to maintain ethical and culturally competent service delivery.

Responsibilities

• Develop and manage individualized case management service plans.
• Perform comprehensive assessments and periodic reassessments.
• Assist clients in applying for health and public benefits.
• Provide recovery support through outreach and workshops.
• Facilitate transportation for clients to medical appointments.
