Job summary

Santa Rosa Community Health seeks a Substance Use Disorder Case Manager (SUD CM) to manage a caseload of SUD/MAT patients within an integrated care setting. You will work with AODS counselors, the MAT team, and other clinicians to support treatment planning, documentation, and referrals.

The role emphasizes collaboration, data upkeep, and patient engagement, with responsibilities across in-person, phone, and virtual visits and a commitment to confidentiality and quality improvement.

Qualifications

• AA degree (Bachelor’s degree preferred) or two years of work experience in a social/human service setting.
• Current certification as an AODS counselor through an organization accredited by the State of California or registration as an AODS counselor intern with capability to complete AODS counselor certification within two years.
• Experience with AODS services and/or principles of harm reduction; MAT experience preferred.
• Bi-lingual (English/Spanish) preferred; strong communication and collaboration skills.

Responsibilities

• Assist with SUD/MAT patient needs, including detailed assessments and development of treatment plans.
• Coordinate with AODS counselors, patients, and providers; ensure accurate intake and documentation in the EHR.
• Administer screening tools for alcohol, drugs, anxiety and depression; track patient engagement in care.
• Support referrals, care coordination across medical, mental health, and community services; maintain confidentiality.
