Job summary

Wedgwood Christian Services is seeking a Case Manager to provide intensive case management for assigned clients in the community. You will coordinate resources, meetings, and education for clients and families on Medicated-Assisted Treatment (MAT) and related topics.

Responsibilities include developing case plans, coordinating with the Clinical Supervisor, and ensuring timely documentation and aftercare. 40 hours per week (24 admin, 16 direct care) with a focus on quality assurance and

Qualifications

• Bachelor’s degree in Social Work, Psychology or another associated human service profession is required.
• Clinical experience with adolescents and/or families is preferred.
• Experience working with an interdisciplinary team in a residential treatment setting is preferred.
• Subscription to the Wedgwood Christian Services Statement of Faith and alignment with agency goals.

Responsibilities

• Develop case management plans for each client with supervision and team input.
• Engage family supports in treatment and educate them on MAT and related topics.
• Coordinate multiple case aspects to meet health and human service needs.
• Develop resource listings for the service area and facilitate quarterly case reviews.
• Track intake through aftercare for reporting and program funding requirements.
• Ensure authorizations and documentation meet contracts and agency requirements.
• Coordinate aftercare to ensure continuity of psychiatric, medical, and therapeutic care.
• Maintain quality assurance compliance and participate in peer supervision and reviews.
• Participate in trainings and ongoing program development.
• Support professional growth of new hires and opportunities for job shadowing.
• Coordinate MAT-related care education with patients and families.
• Follow community partner and PIHP/CMH guidelines for care coordination.
