Qualifications

• Bachelor’s degree in psychology, social work, counseling or related field.
• Master’s degree preferred.
• Licensure or certification (CSAC/ADC/LCSW/LCP/LPC/LSATP) or license eligible.

Responsibilities

• Provide case management and develop individual service plans.
• Coordinate medical appointments and transportation for residents.
• Schedule staff and supervise activities; assist with daily living needs.
• Maintain client records and prepare required reports.
