Job summary

Paycom - ATS in Bellefontaine, OH is seeking a SUD Case Manager to join the Support Services: Alcohol and Other Drug Services team. Responsibilities include working with adults, youths and families to coordinate and deliver mental health and substance use treatment services within a client-centered approach.

The role focuses on developing treatment plans, coordinating care with a treatment court team, and maintaining timely documentation and communication to ensure access to needed resources.

Qualifications

• Education: High School Diploma.
• Licensure: Valid Ohio Drivers License.
• Experience: Two years in mental health/alcohol and drug field; two years in customer service; two years with multi-phone system.
• Certification: CDCA, First Aid, CPR and De-escalation Training.

Responsibilities

• Maintain and coordinate the delivery of evaluations and assessments to meet basic human needs including food, shelter, medical care and mental health services.
• Develop an individualized, comprehensive treatment plan and ensure documentation aligns with OHMHAS standards.
• Provide crisis intervention and stabilization services as needed.
• Assist clients in achieving treatment plan objectives and increasing independence through community resources and training.
