Attain Behavioral Health is seeking a compassionate Case Manager to support clients in substance use disorder and behavioral health treatment programs in Toledo, OH. You will identify barriers, coordinate services, and connect clients with community resources to promote stability and independence.

The role requires strong communication, organization, and crisis intervention skills as part of a multidisciplinary team in a fast-paced, client-centered environment.

Qualificações

• High school diploma or equivalent.
• Chemical Dependency Counselor Assistant (CDCA) credential.
• QMHS III qualification or ability to meet QMHS III requirements.
• Minimum of one year of relevant behavioral health or case management experience.
• Knowledge of community resources and social-service systems.
• Strong crisis intervention, conflict-resolution, and de-escalation skills.
• Ability to remain calm and professional in crisis.
• Valid driver’s license and reliable transportation.

Responsabilidades

• Provide case management services to clients in substance use disorder and behavioral health programs.
• Assess client needs and barriers related to housing, employment, transportation, healthcare, mental health, legal matters, benefits, education.
• Develop and maintain individualized case management/service plans.
• Provide ongoing support and coordination to meet goals.
• Refer clients to community resources (housing, employment, mental health, medical care, transportation, benefits).
• Coordinate care with therapists, peer support, medical providers, and treatment team.
• Use crisis intervention and de-escalation techniques when clients are distressed.
• Maintain a calm, professional, and therapeutic approach in high-stress situations.
• Advocate for client needs and promote independence and self-advocacy.
• Maintain professional boundaries and ethical client-centered services.
• Complete all case management documentation accurately and timely.
• Participate in interdisciplinary team meetings and trainings.
• Stay updated on community resources and regulatory requirements.
• Follow all agency policies and confidentiality requirements.
