Qualifications

• 2+ years in health care navigation, social services, or peer support.
• Experience with documenting in EHR systems and HIPAA-compliant reporting.

Responsibilities

• Connect patients with medical care, insurance, and pharmacy benefits.
• Coordinate resources for food, housing, transportation, and education.
• Facilitate access to physical, mental, and spiritual health services.
• Conduct high-volume outbound calls during care transitions and follow-ups.
• Maintain HIPAA-compliant databases for tracking services and reporting.
