Job summary

Ochsner LSU Health is seeking a Substance Use Navigator to engage ED patients with SUD, linking them to treatment, harm reduction services, and community resources. The role may include variable shifts, overnights, weekends, and holidays to support ED operations.

Responsibilities include trauma-informed bedside engagement, SBIRT, and real-time referrals to MAT and recovery services, with documentation in the EHR and follow-up outreach to ensure continuity of care.

Qualifications

• High School Diploma or GED required.
• Associate's or Bachelor's degree preferred in related field.
• Experience in healthcare, behavioral health, social services, or outreach is preferred.

Responsibilities

• Conduct bedside engagement with ED patients using trauma-informed, person-centered approaches.
• Establish rapport with patients, including those in crisis or ambivalent about treatment.
• Perform SBIRT activities and referrals to treatment.
• Educate patients and families on SUD, harm reduction, and treatment options.
• Facilitate real-time referrals and warm handoffs to MAT providers and community resources.
• Coordinate with ED clinicians, social workers, case managers, and behavioral health teams.
• Document all interactions and referrals accurately in the EHR.
• Assist with follow-up outreach to support continuity of care post-discharge.
• Participate in team meetings and program improvement efforts.
• Stay updated on local resources, programs, and regulatory requirements.
• Support tracking of referral completion and outcomes as required.
