Job summary

Cumberland River Behavioral Health Inc is seeking a Care Coordinator for our residential substance use disorder program. You will join a multidisciplinary team to conduct assessments, crisis intervention, and connect clients with medical, behavioral health, housing and social services.

You will manage intakes, transitions between levels of care, authorizations, and aftercare planning, documenting activities in the treatment plan.

Qualifications

• Minimum Bachelor's degree in Psychology, Sociology, Social Work, Human Services or related field; or a Master's Degree in Human Services with licensure preferred.
• Experience with ASAM levels of care required.
• Submit to and pass a criminal background check and drug screening.

Responsibilities

• Conduct initial screenings and intakes for residential clients entering SUD services.
• Complete transitions between levels of care from 3.5 to 1.0.
• Manage MCO authorizations and reviews for each client.
• Complete referrals to psychiatric, medical, social, educational and other support services.
• Develop aftercare plans with clients prior to discharge and coordinate follow-up.
