Spring Health is seeking a remote SUD Specialty Care Navigator. You will independently manage member care with licensure and advanced skills, coordinate appointments and follow-ups, and collaborate with multidisciplinary teams to support recovery goals.

Responsibilities include engagement, triage, treatment option guidance, and detailed documentation within an EHR. A relevant license and extensive mental health/SUD experience are required.

Qualifications

• Licensed mental health professional with 2+ years of SUD clinical care experience.
• Proficiency in motivational interviewing, triage, ASAM criteria and levels of care.
• Strong communication and collaboration across multidisciplinary teams.

Responsibilities

• Conduct outbound triage and follow-up calls for individuals seeking SUD treatment.
• Assess needs using advanced clinical techniques and provide information on treatment options.
