Job summary

Porch Light Health in Colorado is seeking a Patient Navigator to support access to MAT, OUD/AUD treatment, and behavioral health services. You will guide patients through Medicaid enrollment, navigate insurance options, and arrange transportation and financial assistance to keep patients engaged in care.

This role emphasizes collaboration with care teams, documentation of interactions, and proactive outreach to reduce barriers to treatment.

Qualifications

• Associate’s degree or equivalent in Human Services, Healthcare Administration, Social Work, Psychology, Public Health, or related field preferred.
• Experience in healthcare, patient services, care coordination, or social services preferred.
• Knowledge of insurance navigation and social determinants of health is essential.

Responsibilities

• Assist patients with Medicaid enrollment, renewal, and eligibility.
• Educate patients on Medicaid/Medicare/ ACA options and benefits.
• Coordinate transportation and financial assistance resources to prevent treatment interruptions.
• Engage patients, monitor adherence, and support re-engagement when needed.
• Document interactions and outcomes in accordance with company policies.
