Job summary

BayMark Health Services, Inc. is seeking a compassionate Licensed Substance Use Counselor to support patients in an outpatient setting in Columbus, OH. You will provide individual and group counseling, guide crisis management, and coordinate care with a multidisciplinary team to help patients achieve recovery goals.

Qualifications

• Ohio CDCA, LCDC II/III, LPC, or LSW licensure required.
• Experience with chemically dependent individuals preferred.
• Positive, non-judgmental attitude toward patients.
• Familiarity with EMR systems.
• Satisfactory drug screen and criminal background check.

Responsibilities

• Assist patients through medically assisted treatments and crisis management.
• Conduct individual and group counseling sessions.
• Evaluate progress and collaborate with doctors, nurses and counselors.
• Provide counseling services; coordinate treatment services throughout care.
• Facilitate, establish and review initial treatment plans with each patient.
• Complete bio-psychosocial assessment.
• Develop aftercare and discharge plans.
• Ensure trauma-informed delivery of counseling to all patients.
• Perform other related duties as determined by supervisor.

Education
