Job summary

BayMark Health Services, Inc. is seeking a compassionate Licensed Substance Use Counselor to support patients through medically assisted treatments and develop coping strategies in outpatient care.

You will conduct individual and group sessions, collaborate with physicians, nurses, and other counselors, evaluate progress, and help create treatment and aftercare plans to promote long-term recovery.

Qualifications

• Must possess an Ohio CDCA, LCDC II, LCDC III, LPC, or LSW.
• Experience with chemically dependent individuals preferred.
• Positive, non-judgmental attitude toward patients.
• Familiarity with an EMR system.

Responsibilities

• Assist patients through medically assisted treatments and coping strategies.
• Conduct individual and group counseling sessions.
• Evaluate patient progress and collaborate with clinical team.
• Provide case management: treatment and discharge planning.
• Develop initial treatment plans and aftercare plans.
• Complete bio-psychosocial assessments.
