Job summary

Monarch Healthcare is seeking a Community Support Team Substance Use Counselor to support individuals with mental health and substance use challenges in developing independent living skills and community integration. You will work with a multidisciplinary team to implement service plans and provide education on recovery.

Travel and field work are required, with on-call coverage as needed. A CADC/LCAS or Master’s in Human Services is preferred, with emphasis on person-centered planning and

Qualifications

• Experience with adults with mental health diagnosis and/or substance use disorder required.
• Masters in Human Services or CADC/licensure preferred.
• CADC/LCAS certifications are highly valued.

Responsibilities

• Provide service coordination and develop overall service plans with the team.
• Educate on mental illness, SUDs, relapse prevention, and recovery promotion.
• Apply evidence-based interventions for co-occurring disorders.
• Monitor symptoms and connect individuals to needed services and supports.
• Develop natural support networks and provide individual and group counseling.
• Coordinate with community resources and arrange transportation when needed.
