Job summary

Qualifications

• Certifications: CADC, BLS within 30 days of hire

Responsibilities

• Provides evaluation, counseling and therapeutic services to chemically dependent patients/families assessing social, psychological, emotional, financial, cultural problems and chemical abuse history.
• Develops individual treatment plans based upon information gathered and coordinates the care with the interdisciplinary treatment team.
• Maintains open communication with referral sources, families, or others involved in the patient’s treatment regarding progress, discharge and aftercare plans.
