Job summary

Southern Illinois University Edwardsville (SIUE) is seeking a Care Coordinator in Counseling Services to help students navigate behavioral health care within a stepped‑care model. You will support access, coordinate referrals, and ensure continuity of care with campus and community resources.

The role operates under clinical supervision of an LCSW, with responsibilities including triage, crisis support, and education of practicum students.

Qualifications

• Bachelor's degree required.
• Minimum 1 year in education, training and/or work experience in the area of specialization (Masters may substitute for one year).
• 1+ years experience providing behavioral health case coordination/management, care coordination, and/or resource navigation with knowledge of community mental health resources, insurance systems, and referral processes.

Responsibilities

• Provide behavioral health screening, triage, case management, care coordination, crisis support, and resource navigation.
• Maintain a caseload and provide ongoing follow‑up and continuity of care for students with complex behavioral health needs.
• Help students navigate barriers to care including insurance, transportation, financial, scheduling, and access concerns.
• Coordinate warm handoffs and referrals to Counseling Services, Health Services, community providers, hospitals, and other resources.
• Support students transitioning from higher levels of care by coordinating follow‑up and continuity of care.
• Serve on the university CARE Team and conduct mandated behavioral health assessments.
• Collaborate with campus and community partners to develop coordinated support plans for students of concern.
