Job summary

Penn State Health's Milton S. Hershey Medical Center seeks a Clinical Case Manager for the Heart and Vascular Institute Structural Heart program. The role coordinates care within a multidisciplinary team, focusing on TAVR, M-TEER, T-TEER, WATCHMAN and related workflows.

Responsibilities include mobilizing resources, tracking clinical and fiscal data, and helping develop programs to elevate care quality and outcomes for structural heart patients.

Qualifications

• Bachelor's degree in nursing required.
• Five (5) years nursing experience required.
• Current RN license in Pennsylvania or multistate license via compact.
• AHA BLS certification required.

Responsibilities

• Coordinate care for the structural heart program as part of an interdisciplinary team.
• Mobilize resources and respond to data analyses, identifying variances.
• Contribute to programs that improve clinical quality and fiscal outcomes.
