Job summary

ChristianaCare is currently recruiting for a Physician Assistant or Acute Care Nurse Practitioner to join our Structural Heart Disease Program within the Heart & Vascular Service Line.

The Advanced Practice Clinician will provide medical management and care coordination across inpatient and ambulatory settings, collaborating with a multidisciplinary team to guide patients through evaluation and advanced cardiac procedures.

Qualifications

• Graduate of an accredited Physician Assistant Program or Acute Care Nurse Practitioner Program and board certification.
• License or license eligible in the State of Delaware.
• BLS & ACLS required.
• DEA & Prescriptive authority required.
• Prefer at least 2-3 years of experience working in an acute care setting; previous experience in cardiology and structural heart disease highly desired.

Responsibilities

• Provide comprehensive medical management and care coordination for patients with structural heart disease across inpatient and ambulatory settings.
• Coordinate care across hospital, clinic, and procedural settings to ensure seamless transitions and education for patients and families.
• Engage patients and families in education and shared decision-making.
• Contribute to program growth, quality initiatives, and advancing the team’s efforts in structural heart disease therapies.
