Job summary

Capital Cardiology Associates is seeking a highly organized Patient Care Coordinator to join our Structural Heart Program. This full-time, Monday–Friday administrative role focuses on coordinating care for patients with structural heart conditions, communicating with patients, physicians, hospitals, and insurers, and maintaining accurate records.

Qualifications

Prior administrative or medical office experience strongly preferred.
Experience with insurance coordination is a plus.
Comfortable navigating electronic medical record (EMR) systems.

Responsibilities

Provide administrative support for the Structural Heart Program and care team
Communicate with patients by phone and help with questions and care coordination
Work with medical insurance companies regarding services, coverage, and requirements
Coordinate and facilitate patient appointments, testing, procedures, and other care aspects
Maintain accurate documentation in the electronic medical record
Communicate with physicians, clinical staff, hospitals, and healthcare partners
Track outstanding items and follow up to prevent gaps
Manage multiple priorities with accuracy and organization
Provide compassionate, professional service throughout the care journey
Perform other duties to support the program
