Harborview Medical Center's Stroke Center has an outstanding opportunity for a Registered Nurse (RN) - Nurse Navigator. The Stroke RN Navigator plays a vital role in education, complex care coordination, and collaboration across the stroke care continuum from admission through discharge and outpatient follow‑up.

This full‑time role requires a BSN and Washington RN license, with at least 3 years of RN experience in neurology or care coordination.

Qualifications

• Bachelor of Science in Nursing (BSN).
• Current RN license to practice in the State of Washington.
• Minimum 3 years of RN experience, ideally in neurology/neurosurgery/critical care or care coordination.
• Strong communication skills and ability to engage with multidisciplinary teams.

Responsibilities

• Deliver patient education at the bedside and support family engagement.
• Coordinate inpatient care with care management, physicians, and therapy teams.
• Facilitate discharge planning and post-discharge follow‑up with patients and families.
• Participate in stroke program initiatives, quality improvement, and data collection.
