Job summary

UBMD Neurology in Buffalo, NY is seeking a Registered Nurse for the Vascular Neurology/Stroke program. The role focuses on patient navigation, care coordination, and clinical triage, with cross-coverage in neurology workflows as capacity allows.

The RN will monitor stroke discharges, coordinate post-visit care, manage calls and portal messages, and collaborate with providers to ensure continuity of care. Opportunity for procedural involvement may arise.

Qualifications

• Current New York State RN license in good standing.
• Graduate of an accredited nursing program.
• Clinical nursing experience required.
• Neurology/stroke care coordination or triage experience preferred.
• Strong assessment, communication, and organizational skills.
• Ability to prioritize clinical concerns and upscale urgent needs.
• Experience with electronic medical records and patient workflows preferred.

Responsibilities

• Monitor stroke discharges and ensure outpatient follow-up scheduling.
• Provide stroke patient navigation and prevent loss to follow-up.
• Coordinate post-visit care, including testing and referrals.
• Triage calls, portal messages, and clinical tasks with changing symptoms.
• Escalate urgent concerns to the appropriate provider.
• Provide patient education and assist with care coordination.
• Offer RN triage and support for other neurology subspecialties when possible.
• Collaborate with providers, APPs, nursing, admin staff, and outside facilities.
• Assist with office-based procedures as Vascular Neurology programs expand.
