Job summary

Harborview Medical Center’s Stroke Center is seeking a Registered Nurse (RN) – Nurse Navigator to coordinate care throughout the stroke continuum, from admission through discharge and follow-up.

You will educate patients and families, perform inpatient navigation, discharge planning, and post-discharge coordination, while contributing to quality improvement with a multidisciplinary team. A BSN and WA RN license plus at least 3 years in neurology/acute care are required.

Qualifications

• BSN required and WA RN license in good standing.
• Minimum 3 years RN experience in neurology/critical care/care coordination.
• Ability to collaborate with multidisciplinary teams and communicate effectively.

Responsibilities

• Deliver bedside stroke education to patients and families.
• Coordinate inpatient flow and discharge planning with care teams.
• Perform post-discharge follow-up calls and care handoffs.
• Assist with quality improvement efforts and stroke-related metrics.
