Candidates must have clinical experience, be enrolled in a BSN program, and obtain SCRN/CCRN certification. Proficiency in Microsoft Office and a current Louisiana RN license are also required.

Qualifications

• Four years acute clinical experience required; at least 1 year working with a Stroke patient population.
• Proficiency with Microsoft tools (Excel, Word, PowerPoint).
• Current and unrestricted Louisiana RN license.
• Proficiency with Microsoft Excel, Word, and PowerPoint.
• Current and unrestricted Louisiana RN license.
• Current BLS certification.

Responsibilities

• Conduct patient rounds utilizing clinical expertise to aggregate, assess and interpret information.
• Collaborate with multidisciplinary care team to promote optimal outcomes for stroke patients.
• Identify areas of improvement; use data to develop educational programs.
• Coordinate and participate in stroke performance improvement/patient safety forums and committees.
• Support new stroke initiatives by identifying and overcoming barriers to clinical care.
• Assist with preparation for Joint Commission certification surveys.
• Serve as an active member of stroke workgroups and committees and participate in strategic planning for the Stroke Program; provide input to meeting agenda, report on significant trends and outstanding items.
• Serve as a resource/mentor to bedside staff within the stroke service line.
• Collaborate with multidisciplinary care team to promote optimal outcomes for stroke patients.
• Partner with stroke program providers to facilitate optimal care supported by guidelines.
• Identify, validate and analyze issues, and initiate follow-up to assure loop closure to improve clinical outcomes and patient care.
• Identify areas of improvement; use data to develop educational programs and action plans to address quality issues and ensure follow-through of developed plans.
• Facilitate stroke patient education and coordinate follow-up care to decrease further stroke complications and readmission rate.
• Counsel and train staff members to increase awareness of adverse trends and participation in interdisciplinary process improvement.
• Instruct others on pertinent standards and guidelines; ensure these standards and guidelines are adhered to.
• Other duties as assigned.

Job Description

The Stroke Navigator assists the Stroke Medical Director and Stroke Program Director to improve the quality/delivery of ischemic and hemorrhagic stroke care throughout the continuum of care. He/she works in collaboration with program directors, data analysts, department leaders, staff and patients to develop and support the stroke program’s performance improvement initiatives. The Navigator also assumes responsibility and accountability for the management of resources to achieve efficient, high-quality outcomes for each patient, including support for interdisciplinary and cross-facility collaboration. In addition, he/she identifies opportunities to streamline care practice, using LEAN methodology, through development of evidence-based guidelines and protocols.
