Job summary

Cynet HealthStaff is seeking an experienced RN to serve as Nurse Navigator in a certified stroke program. The role provides individualized guidance, education, coordination, and support to stroke patients, families, and caregivers across the continuum of care, from acute hospitalization through rehabilitation and follow-up.

Responsibilities include inpatient stroke education, supporting prevention strategies, facilitating transitions of care, and collaborating with staff to close care gaps.

RN - Nurse Navigator

Experience: Minimum of 5 years of recent relevant acute care experience for stroke patients or as a nurse navigator assisting patients with post-discharge barriers.

License: Registered Nurse License

Certifications: BLS (Basic Life Support), NIH Stroke Scale Certification (NIHSS), Certified Neuroscience Registered Nurse (CNRN) or Stroke Certified Registered Nurse (SCRN) preferred.

Must-Have:

• Experience as a stroke coordinator in a certified comprehensive stroke center or a primary stroke center performing mechanical thrombectomies.
• Effective communication skills related to stroke education for both staff and patients.
• Comfortability with chart review to identify gaps in stroke care.
• Ability to work collaboratively with the primary nurse to close care gaps.

Description:The Stroke Nurse Navigator is a professional Registered Nurse who provides individualized guidance, education, coordination, and support to stroke patients, families, and caregivers across the continuum of care.This role serves as a key resource within a certified comprehensive stroke center.The navigator utilizes specialized knowledge and experience in the care of stroke patients to improve outcomes and reduce barriers to care.This position enhances the patient experience and promotes optimal clinical outcomes through seamless care coordination from acute hospitalization through rehabilitation, outpatient follow-up, and community reintegration.Responsibilities include providing comprehensive inpatient stroke education to patients and families.The navigator reinforces stroke prevention strategies, risk factor modification, medication adherence, and recognition of stroke warning signs.The role also leads and participates in community outreach initiatives focused on stroke awareness, prevention, education, and access to resources.The navigator participates in the development, implementation, monitoring, and sustainability of stroke program initiatives, quality improvement activities, regulatory compliance efforts, and performance measures associated with maintaining certification.Through collaboration with various healthcare partners, the navigator works to improve transitions of care, support evidence-based stroke management, and promote positive outcomes for patients and providers.
