Job summary

Cynet HealthStaff is seeking an experienced RN to serve as Nurse Navigator in a certified stroke program. The role provides individualized guidance, education, coordination, and support to stroke patients, families, and caregivers across the continuum of care, from acute hospitalization through rehabilitation and follow-up.

Responsibilities include inpatient stroke education, supporting prevention strategies, facilitating transitions of care, and collaborating with staff to close care gaps.

Qualifications

• Experience as a stroke coordinator in a certified Stroke Center.
• Effective communication with staff and patients regarding stroke education.
• Ability to review patient charts to identify gaps in stroke care.
• Ability to work with the primary nurse to close care gaps.

Responsibilities

• Provide comprehensive inpatient stroke education to patients and families.
• Coordinate transitions from acute hospitalization to rehabilitation and outpatient follow-up.
• Identify gaps in stroke care and collaborate with teams to close them.
• Lead and participate in community stroke awareness and outreach initiatives.
