Job summary

Beacon Health System is hiring a full-time, day-shift Stroke Pt Navigator for Memorial Hospital in South Bend, IN. This role reports to the Director of Cardiovascular and Stroke Services and focuses on coordinating patient care, education, and data collection to meet stroke center standards.

The Nursing Admin Patient Care Department involves collaboration with physicians and hospital staff to provide comprehensive, patient-centered stroke care and to support quality improvement initiatives.

Qualifications

• Current RN license in Indiana is required.
• Bachelor's degree in nursing (BSN) is preferred.
• Experience in neuro or stroke care is desirable.

Responsibilities

• Coordinate care for stroke patients and monitor in-house patients.
• Collaborate with physicians and departments to maintain stroke center standards.
• Participate in quality improvement and data collection for stroke outcomes.
• Assist with stroke education for patients and families.
