Job summary
Stamford Health is seeking a Full‑time Stroke/Neurosurgery Nurse Navigator to coordinate multidisciplinary services and improve care quality for stroke and neurosurgery patients. The RN will lead patient education, ensure timely access to services, and collaborate with the Stroke Center to meet quality standards.
The role involves data collection, performance monitoring, and liaison work with physicians and families to support ongoing care and discharge planning at Stamford Health.
Qualifications
- Requirements include nursing degree and CT RN license with BLS/ACLS certifications.
- Preferred Stroke Certified Registered Nurse (SCRN) or Certified Neuroscience Registered Nurse (CNRN).
- Minimum ongoing education and annual contact hours.
Responsibilities
- Coordinate multidisciplinary stroke/neurosurgery care and improve quality.
- Support data collection for process and quality improvement and monitor performance trends.
- Collaborate with Neuroscience Institute to enhance patient care and safety.
- Educate patients and families through diagnosis and treatment phases.
- Navigate patients from diagnosis to treatment ensuring timely services.
- Communicate patient status and discharge information to families.
