Job summary

Yale-New Haven Health System is seeking a Stroke Coordinator to provide leadership, program management, and strategic direction for the Stroke Center. The role focuses on multidisciplinary care, regulatory compliance with Joint Commission standards, and ongoing quality improvement to enhance patient outcomes.

The position collaborates with medical leadership and education teams to deliver stroke education and ensure high patient and family satisfaction.

Qualifications

• Masters degree in Nursing or enrollment with expected graduation within one year.
• CT RN license or CT APRN license required.
• Experience in neuroscience, cardiovascular, stroke, or critical care nursing is essential.
• Leadership experience in a healthcare setting is preferred.

Responsibilities

• Lead inter-disciplinary team to develop best practices and guidelines.
• Coordinate compliance readiness for Joint Commission certification.
• Oversee quality improvement processes for the stroke program.
• Collaborate with physicians, nursing educators, and staff to identify educational needs.
• Support telestroke program as clinical liaison and provide nursing expertise.
• Create and manage stroke call schedules in collaboration with leadership.
