Job summary

Yale NewHaven Health is seeking a Stroke Coordinator to provide leadership and program direction for the Stroke Center, ensuring high-quality care and regulatory readiness. You will oversee multidisciplinary teams, coordinate education, and support initiatives to improve patient outcomes.

Qualifications

Master's degree in Nursing; enrollment in a master's program acceptable if expected graduation within one year.
Minimum 3 years of neuroscience, cardiovascular, ED, stroke rehab, or critical care nursing; leadership preferred.
CT RN or CT APRN license.
Strong leadership, collaboration, communication, and analytical skills; ability to develop guidelines and educate staff.

Responsibilities

Leads an inter-disciplinary team to develop best practices and evidence-based guidelines.
Maintains compliance readiness for Joint Commission certification and related documents.
Drives performance improvement through annual plan development.
Assess educational needs of patients, families, and the community with physician/nursing input.
Promotes patient, family, and provider satisfaction through quality care.
Acts as clinical liaison for the telestroke program and coordinates with leadership.
