Job summary

West Jefferson Medical Center in Louisiana seeks an RN Stroke Program Coordinator to lead a department-wide initiative, coordinate accreditation processes, and advance a regionally recognized stroke program. The role centers on interdisciplinary care, education, and data-driven improvement to enhance patient outcomes.

You will design pathways, educate staff, and partner with hospital leadership to ensure regulatory compliance, standardized care, and community outreach.

Qualifications

• Advanced clinical, educational and managerial skills in interdisciplinary care for stroke patients.
• Ability to design and implement clinical pathways for stroke care.
• Strong leadership and staff education capabilities.

Responsibilities

• Facilitate hospital-wide stroke activations with rapid triage and treatment.
• Educate staff and coordinate with physicians, nurses, therapists and case managers.
• Develop and evaluate patient education materials and discharge teaching.
• Lead quality improvement initiatives and data utilization for stroke care.
• Collaborate with nursing leadership to optimize processes and standards of care.
