Job summary

LCMC Health is seeking a professional to lead the stroke program at West Jefferson Medical Center. The role requires advanced clinical, educational and managerial skills to design and implement stroke pathways, coordinate interdisciplinary care and improve patient outcomes.

You will educate staff, collaborate with providers, participate in committees, develop patient education materials and drive data collection to measure program performance.

Qualifications

• Advanced clinical, educational and managerial skills in interdisciplinary stroke care.
• Ability to design and implement clinical pathways for stroke patients.
• Experience educating staff and leading quality improvement in a hospital setting.

Responsibilities

• Facilitate interdisciplinary team involvement across the continuum of care.
• Lead hospital-wide stroke activations and rapid triage.
• Participate in stroke rounds and coordinate progress and discharge planning with the team.
• Educate staff and the public on stroke awareness and prevention.
• Develop and evaluate patient education materials and resources.
• Drive stroke program data collection and analysis for program improvement.
• Collaborate on standards of care, protocols and staff development.
