Job summary

Jobtailor is seeking a Stroke Program Coordinator to lead clinical stroke initiatives across our facilities. You will coordinate stroke programs, guide Stroke Champions, and serve as a clinical resource for care teams.

You will analyze outcomes, prepare performance reports for regulatory reviews, and support community education while ensuring accurate data submission to Joint Commission.

Qualifications

• Current licensure in one of the listed professions (SLP/OT/PT/LPN).
• Basic Life Support (BLS) certification within 30 days of hire.
• One year of experience in a neurology-related position.

Responsibilities

• Coordinate the facility stroke program.
• Utilize and manage Stroke Champions on every unit related to stroke care.
• Serve as a resource to the hospital for care of stroke patients.
• Facilitate community outreach activities related to stroke awareness and education.
• Complete ongoing staff educational opportunities for all levels involved with stroke care.
• Collect and analyze data regarding outcomes of the stroke program.
• Analyze data, prepare reports for Stroke Performance Improvement reviews.
• Submit required data to the Joint Commission and other regulatory agencies.
• Evaluate effectiveness of patient teaching.
