Job summary

Qualifications

• 5 years RN experience in an acute setting.
• Current Illinois Registered Nurse license.
• Strong data coordination and chart abstraction skills.

Responsibilities

• Maintains concurrent data abstraction of stroke patient records.
• Provides feedback to nursing staff and Neuro Program Coordinators based on data.
• Presents stroke data at Stroke Committee Meetings and CPIC meetings.
• Maintains post-discharge call back program including EMMI for all stroke patients.
