Job summary

Southern Illinois Healthcare (SIH) is seeking a registered nurse to lead concurrent chart review and data abstraction for stroke patients. You will support the Neuro Program Coordinators, maintain the discharge callback program, and oversee the EMMI initiatives tied to stroke care.

Ideal candidates have an Illinois RN license and at least 5 years of acute RN experience. This on-site role emphasizes data accuracy, feedback to staff, and quarterly presentations to the Stroke Committee.

Qualifications

• Bachelor's degree in nursing preferred or associate's degree in nursing.
• Current Illinois RN license.
• Minimum 5 years RN experience in an acute setting.

Responsibilities

• Maintains post discharge call back program including EMMI for all stroke patients.
• Primarily responsible for concurrent data abstraction of stroke patient records.
• Provide feedback to nursing staff and Neuro Program Coordinators to improve Stroke Program.
• Presents stroke data bi-monthly at Stroke Committee Meetings.
• Reports stroke data at CPIC meetings.
