Job summary

Memorial Hospital of Carbondale, part of Southern Illinois Health, seeks a Registered Nurse to coordinate stroke program data. You will perform concurrent chart reviews, abstract data for stroke patients, and support the EMMI discharge back-call and related data processes.

The role reports to Neuro Program Coordinators, provides feedback to nursing staff, presents stroke data at committee meetings, and helps maintain quality standards through data-driven insights.

Qualifications

• 5 years RN experience in an acute setting.
• Current Illinois Registered Nurse license.
• Strong data coordination and chart abstraction skills.

Responsibilities

• Maintains concurrent data abstraction of stroke patient records.
• Provides feedback to nursing staff and Neuro Program Coordinators based on data.
• Presents stroke data at Stroke Committee Meetings and CPIC meetings.
• Maintains post-discharge call back program including EMMI for all stroke patients.
