Job summary

CarePlus New Jersey seeks a qualified behavioral health professional to provide family and individual support, case management, assessments, intervention planning, education, and aftercare planning. The role ensures program goals are met and team duties are fulfilled.

You will work within an interdisciplinary team, document services, and contribute to care coordination and outcomes documentation.

Qualifications

• Master's degree in a behavioral health discipline from an accredited college or university.
• 1–2 years of clinical experience in the mental health field.
• Ability to respond to clients for crisis intervention.
• Provisional/Professional Licensure to practice counseling/clinical social work (LAC, LSW, LPC, LCSW, LMFT).
• A valid driver's license and you must be insurable by the agency's automobile insurance.

Responsibilities

• Assess and evaluate clients at intake and during ongoing treatment and at discharge/transfer.
• Respond to clients for crisis intervention.
• Contribute within an interdisciplinary team and document assessments and plans.
• Provide leadership within the team and coordinate care.
• Formulate psychosocial treatment plans with measurable goals and timelines.
• Plan, communicate, and document discharge/aftercare/transfer plans.
• Document assessments and progress in the clinical record with time/date stamps.

Job Summary

Provide supportive family and/or individual contacts, case management, assessments, intervention planning, client and/or family education, and aftercare planning. Assures program goals and objectives are accomplished by fulfilling job and team responsibilities as assigned.

What You Will Do

• Assess and/or evaluate clients at intake and/or admission, during ongoing treatment and/or service delivery, and at discharge/transfer


• Respond to clients for crisis intervention


• Function within an interdisciplinary team in the assessment and/or evaluation of clients, and to articulate verbally and in written documentation the integration of client history and assessment information.


• Provide leadership within the team, document services in the clinical record, engage in inter-organizational care coordination, and document service outcomes


• Formulate psychosocial treatment plans, with relevant goals, behaviorally-measurable objectives that serve as indexes for evaluating treatment progress, and realistic time frames for achievement


• Plan, communicate, and document discharge/aftercare/transfer plans for clients completing treatment and/or accessing other services in the continuum


• Document assessments, treatment plans, progress reports, and discharge summaries in the clinical record, with all entries authenticated and time/dated

What Makes You an Ideal Candidate

• Master's degree in a behavioral health discipline from an accredited college or university
