Job summary

Detroit Recovery Project is seeking a compassionate Case Manager to provide direct support to participants and connect them with essential resources. You will screen individuals, develop individualized plans, and link them to community services while maintaining a minimum caseload of ten clients.

Under the Prevention Project Coordinator, you will facilitate prevention education, document progress, and collaborate with community partners to promote healthy, sustainable lives for participants.

Qualifications

• Bachelor's degree in Human Services or related field.
• Minimum of 1 year of professional case management experience.
• Certified HIV Tester in the State of Michigan.
• Valid and unrestricted Michigan Driver's License.
• Proficiency with Microsoft Office Suite.

Responsibilities

• Identify participant needs and develop strength-based independent living plans with measurable goals.
• Provide referrals and link participants to community resources and services.
• Maintain a minimum caseload of ten clients.
• Screen participants to determine eligibility for case management services.
• Provide warm handoffs to promote continuity of care and successful service connections.
• Coordinate prevention services with outside organizations and community partners.
