Job summary

South Middlesex Opportunity Council, Inc. is seeking a Street Outreach/CE Case Manager in Framingham, MA. You will provide street outreach, perform comprehensive needs assessments, and engage clients toward coordinated entry and housing resources.

Travel throughout the western Massachusetts region is required. You will work with partner agencies to ensure safety, health, and access to services, while maintaining confidential client records.

  • Flexible schedule and willingness to travel regionally.

Summary: Provide street outreach, case management, and Coordinated Entry (CE) assessments to qualifying or assigned clients, a combination of those living outside and areas not meant for habitation. Target population include those who are living outside and those identified by Police Officers or other community providers as needing services. Work closely with other community-based providers to ensure immediate and long-term health and safety needs of the participants are addressed. Work to engage clients in Case Management and refer to appropriate region and mainstream benefits. Develop/monitor plan to assist in improved recovery, housing status and overall health. Travel throughout the CE west region is required.

Primary Responsibilities
  • Provide street outreach and implement engagement strategy with all new clients referred to the program and conduct a comprehensive service needs assessment with each client.
  • Provide outreach at the Day Center as needed and according to a schedule. Attend meetings at SMOC, COC, The HUB, and others as required by contract. Participate in regional CE committee meetings.
  • Guarantee data is accurately collected and submitted in VESTA
  • Develop, implement, and monitor Individualized Service Plan with each participant.
  • Ensure coordination of service delivery on behalf of each client. Work in collaboration with all other service providers working with the participant.
  • Build working relationships with other area agencies within the CE west region.
  • Assist individuals with medical emergencies in finding detoxification services and appropriate treatment.
  • Keep accurate client records and provide monthly written updates including client updates, demographics, barriers and new resources, etc.
  • Assist individuals in accessing new resources and supports which will lead to safer choices and reduce risky behaviors that may include panhandling, prostitution, and substance use.
  • Engage all clients by understanding and addressing their needs whether within or outside the scope of work.
  • Attend & participate in engagement team meetings as requested and communicate effectively with client and staff in other areas.
  • Work collaboratively with the CE Housing Navigator and Program Manager to ensure program compliance.
  • Maintain confidentiality of client, employee and agency information in accordance with federal and state laws and funder requirements. Ensure compliance with program/department, agency and/or funder requirements, as well as, SMOC policies & procedures.

Knowledge and Skill Requirements
  • Bachelor’s Degree or 3 years of experience in Human Services or related field.
  • Sensitivity to low-income people of diverse backgrounds.
  • Ability to work well as a member of a dynamic team.
  • Valid license and reliable transportation.
  • Assessment, advocacy and case management skills.
  • Flexible schedule required.
  • Bilingual preferred.
Organizational Relationship

Directly reports to the Supportive Housing Program Manager.

Physical Requirements

Must be able to conduct street outreach which often may include long periods of walking and standing while speaking with clients.

Working Conditions

Frequent travel throughout the Western region of Massachusetts is required. The Street Outreach Worker will work at multiple office spaces, such as, the Day Center in Framingham and the Satellite office in Franklin, as well as, 7 Bishop Street in Framingham. As part of the responsibilities of this position, the Case Manager, CE Outreach will have direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Housing and Livable Communities. A successful background check is required.


Monday through Friday: 9:00AM - 5:00PM. Includes a 1-hour unpaid lunch break.
35 Hours per week.
