Job summary

South Middlesex Opportunity Council (SMOC) in Framingham, MA seeks a full-time Care Coordinator to ensure client safety and access to needed services. You will work within a clinical team, coordinating medical behavioral services, MAT, and referrals while maintaining client files.

The role requires a Bachelor’s degree or 5 years in social services, strong communication, and a valid driver's license. Shifts include evenings and weekends, with a 9:00 AM to 5:00 PM schedule and a 35-hour workweek.

Qualifications

• Bachelor's Degree or 5 years relevant work experience in social services, recovery or mental health, or housing services.
• Understanding of addiction as a disease concept.
• Knowledge of 12 Step programs.
• Understanding of mental health diagnosis and trauma.
• Strong verbal and written communication skills.
• Must have valid Driver’s License and reliable transportation.

Responsibilities

• Provide program coverage, supervise daily/nightly and weekend activities.
• Coordinate medical behavioral services through insurance.
• Coordinate MAT and psychiatric services.
• Make referrals for external resources including medical, dental, health benefits, job training, Mass Rehab, career center, etc.
