Job summary

HCRS (Health Care & Rehabilitation Services of Southeastern Vermont) seeks a Street Outreach Care Coordinator to engage individuals experiencing homelessness and housing instability in nontraditional settings. You’ll assess needs, connect clients to medical, behavioral health, and housing services, and advocate to reduce barriers to care.

Qualifications

• Bachelors in social work or related field preferred with experience in outreach.
• Experience with trauma-informed, client-centered service delivery.
• Ability to work independently in community settings and coordinate services across systems.

Responsibilities

• Conduct proactive outreach to individuals in streets, encampments, and nontraditional settings.
• Build rapport and trust with clients reluctant to engage with services.
• Provide ongoing outreach and engage clients toward stabilization and service connection.
• Develop individualized service plans and coordinate access to health, housing, and supports.
• Advocate to reduce barriers and document outreach and referrals meticulously.
• Collaborate with community partners and participate in care planning meetings.
